The patient, a 59-year-old woman with no relevant past history, presented to the outpatient clinic with persistent pain in the right upper quadrant of her abdomen over the past month. The patient also mentioned fatigue and progressive abdominal distension during further questions. She did not report any other symptoms like changes in her bowel habits, abdominal pain, or weight loss. For HB normocytic anemia, laboratory testing was important: 9 (13-17 g/dL); VGM: 90 μm³ (80-100 μm³), moderate thrombocytopenia 98.000/mm3 (140-440 000 mm3), hypofibrinogenemia with fibrinogen level of 1.09 g/L (2-4 g/L), and delayed INR and the liver board was without range. She underwent an abdominal ultrasound, which first revealed a large, heterogeneous, hyperechoic liver mass (18 x 17 x 15 mm) with multiple hypoechoic tubular images of vascular appearance echogenicity occupying the right liver. After that, an MRI was performed, which confirmed the diagnosis of a massive hemangioma that appeared to be associated with kasabach- Merritt syndrome. The patient was treated with corticotherapy and salicylic acid, but the results were not satisfactory; after a few weeks, he had surgery, and the exploratory laparotomy revealed a massive hypervascular angioma that covered the entire right liver. The right liver with the mass was removed via right hepatectomy without capsular effraction or angioma rupture. There were no bleeding issues during the surgery or the recovery process. The excision's anatomical and pathological examination revealed a benign cavernous hemangioma. With no recurrence of angiomatous disease and the Kasabach-Merritt syndrome disappearing, the evolution was favorable.
Hemangiomas in the liver have been shown to grow to astonishing sizes and may be the cause of difficult and rare complications like Kasabach-Merritt syndrome. Surgeons, internists, and radiologists alike need to be aware of these possibilities in order to correctly diagnose and treat them when they arise. It is difficult, but essential for treatment, to distinguish between Congenital (HC) and Infantile (HI) hemangiomas. It is difficult to tell the difference between infantile and congenital hemangiomas, but it is necessary for the right treatment. This retrospective study sought to compare and describe the epidemiological, clinical, and treatment characteristics of congenital and infantile hemangiomas discovered at a tertiary care hospital over a three-year period. The type of response was unaffected by variables such as the patient's gender, location, depth, FIV gestational age, and treatment method. This contrasts with the literature, where the superior response is found in superficial hemangiomas. Some authors have recently proposed that combining timolol with lacer could increase its effectiveness to a level comparable to that of propranolol. Atenolol and nadolol might be an option for patients who can't take propranolol. This is based on studies that show that they are no worse than propranolol and have a better safety profile.
